They were custom made by a local company here in Utah.

I don't have the price but these doors were built in 2009, and the price then, wouldn't be accurate today. Most door manufacturers should be able to recreate these doors. The best thing to do is contact a local company and ask for a quote. Pricing can also change depending on quantity and materials used.
