When you have custom cabinets built around the range hood like this, do you install a regular wall mount range hood and then cover it with cabinets or is there a special range hood that is intended to be covered (similar to a built-in fridge)?

There are several options available from the appliance stores. Ask for "Hood Liners" They are available with internal or external mounted fans. If you have the ability to mount an external fan on the outside wall it will greatly reduce the fan noise level in the kitchen.
