Trim away the damaged section cleanly so you don't run the risk of borers or other diseases setting in, and let the tree re-branch. See if you can encourage even branching. You'd be surprised how resilient apple trees are. Especially the Ein Shemmer.

Choose one of the shoots to be the new trunk and pinch back other growth just enough to sustain dominance of that trunk. As your new trunk grows, remove all new shoots more than a third its diameter coming off it. Do nothing else until tree begins to fruit heavily.

Once one of the shoots starts, should I cut the original trunk off just above it? The new growth so far, is only a few inches above the graft. Or should I wait to see if a shoot starts closer to where the damage begins which is about 3 ft from the ground.

If that's where the shoots have started the rest of the trunk is probably fried. Wait a few days and then cut just above existing shoots.
