Sick Phal Orchid?

I recently bought an orchid at Lowe's, about a month ago actually, my previous one had died due to a tragic incident involving my dog eating the majority of it after knocking it off the table. I've been doing everything the same with this one that I did my last, my last lived for quite a few years...

The pot which is a square glass container 4x4" with moss in it (same as last). The roots are visible through the moss so I didn't think lack of air to the roots would be an issue. I did though, gently lift the plant out of the container and found that the moss was so tightly packed in there that I could fill 2 handfuls of it and STILL have plenty in the container. Also with the moss, I let it dry completely in between watering.

However, in the past 3 days 2 leaves have yellowed at the base and fallen off and it only has 3 blooms left. I assumed that the blooms dropping off was just the end of the bloom cycle.

The Phal doesn't get any direct sunlight, just morning and afternoon indirect light. Though it is in the kitchen which is really bright because of the large windows we have. This is the only thing that has changed due to the fact that I've moved since the last orchid.

I'm looking for any help here as I am at a loss. I did take pictures and can post them if anyone wants to see them but with how descriptive I've been I wasn't sure.

Thank you folks for any help in advance!

Binx
