Hi there. Hm. That one looks etiolated (reaching out for the light). What you do next will depend on what "look" you want your plant to have.

If it was my plant, I would definitely whack it just above the caudex for a closer, more compact growth. Cut diagonally across the main stem and wait for new growth. When you see the bumps, put it in a spot where it will get the maximum amount of sun for as long as possible. Adeniums need intense sun if you want to keep it compact. With enough sun, warmth and proper feeding, I found that the plant will gain girth instead of just height.

I'm using a fast draining cactus mix for soil. Should I change the soil out when I cut it off?

Frankly? I hate that soil mix lol

First of all, it retains way too much water for most cactus and succulent plants. I think your adenium will be okay if you don't want to bother. But if you are in the mood for it, you can still use that mix, just make it a half-and-half, i.e., half cactus mix and half perlite (not vermiculite, the two are not interchangeable when we're talking about moisture control). That way, the amount of water at the bottom of your pot will be less. Ish.
