Filters overflowing

I have two Tetra Whisper hang-on-back filters for my 20g aquarium. Both are overflowing - water flows up the rear of the filter box and then cascades down the sides without going through the filter media. You'd think it might be because the media need cleaning, but cleaning doesn't seem to solve the issue. Once I pulled both filters out and took them apart and scrubbed everything down, and that helped for a couple days but that was it. The media are never particularly dirty when I clean them during water changes.

Anyone got any ideas on why filters do this?
