Ken is correct. It is almost surely voles.

Chris over at Hallson's has a superb solution. Get some plastic containers (Glad lock, etc) with the snap on lids. Cut a hole in the side of the container, about 3/4 - 1 inch in diameter in one end. Place a rodent poison bait block in the container and snap on the lid. Place the baited container, opening near the ground near the holes in the dirt. The rodents will go in to eat the bait, but can't remove the entire block. Also it prevents larger critters (dogs, kids, etc) from easy access to the poison.

The only good vole is a dead vole.
