High Nitrate

I have a roughly 250 gallon pond with four Koi. It has a basic biolgical filter system with a waterfall for aeriation. For some reason, the Nitrate and Amonia levels seem to always be too high. I condition all water before putting it in the pond and the PH is fine. What could be causing the high Nitrate and Amonia. Could bird waste falling in the pond cause this? Also, in the past there has been falling small green seed type things from a vine which grows on a wall near the pond. These falling green things which require me to skim the pond daily have already made the water so brown at one point I had to compleely drain the pond and refill. Is the brown water a whole other issue? Any help would be appreciated as I wan the best environment for my Koi. Thanks.
