what did i do wrong?

My mom recently passed away and I wanted to transplant some of her iris plants to my garden for sentimental reasons. So, several weeks ago I went and dug up several of her smaller iris plants. They werent part of the large colony but were by themselves so i didnt have to break them off. I read up on iris planting before doing this, so when i planted them in my garden bed I did the little mound and spread the roots out and I made sure they werent too deep. After several days the leaves started turning yellow and brown and were dying off so I trimmed them. And the picture is what it looks like today. The only thing i can think of is that its not the right time of year to be moving them? Any suggestions would be greatly appreciated!
