they're relatively sturdy plants for the most part (have to be to make it millions of years without evolving past spore production as a form of procreation!) and I just lift them with a garden fork (checking for runners as I go- some times you'll get two crowns for the work of one) and dig them in to their new home.

the big trick is to plant them into a similar environment to the one you dug them from, to reduce transplant shock.
