Have you considered hiring a landscape designer?

Finding a nursery that can provide all three may be difficult. With a plan you can shop around for the best price. I did a google search and there are not too many garden center/ nurseries in Boyertown.

I saw a few.
Tell them what you want to do and ask for an estimate for a landscape plan. Ask them to show you examples of their work (plans) and refereces. Go to local nurseries and ask if they do business with them and if they are reputable.

They should provide a scale drawing of your site with all plants easly identified and also a planting list with the names and descriptions of all the plants. Make sure that the plants can be purchased locally.

A plan is your best bet. Once you have the plan you can shop anywhere.

cost could vary between $400-500 --- and up. It depends on lots of factors.
Sounds like a lot of money, but imagine if you home builder built you home with out a plan and it was his first time - Also a good landscape can add 10-15% to the value of your home
