Rose Bush Identification

Hi, I am hoping someone can help me with the identification of a rose bush that I have acquired a piece of. The original rose bush was planted by my great grandmother who was born in 1855 and died in 1944. The bush grows between 5-6 feet tall. It doesn't seem to put out a lot of succors though every once in a while it will put one out. I have a few photos of the rose.

I am hoping someone can identify this rose as I do a family history and would like to have information for future generations about this rose bush in it. I would also like to be able to find out if I am treating the slip of it I have correctly. It is doing very well. It does seem to sprawl a bit. Thank you for any help you may be able to give me.
