my salvia looks flat

I don't know exactly what kind of salvia I have, but it has dark blue flowers early in summer that fall off easily. My plant looks like it collapsed...like it was pushed down in the middle and you can see where the stems come out of the ground. The flowering part of the stems still bend upward though.

Is this considered sprawling? Do I need to support the plant so it doesn't do this next year? Should I prune it?

It would be so nice if my salvia didn't do this! I feel like I should have planted it somewhere else!
