Spots on tomato plant leaves

I just started growing these tomato plants from seeds a little over a month ago. The seeds are from cherry tomatoes from a Costco vegetable dish. The spots first show up as just a few little white spots about the width of a toothpick with tiny little black dots on and around the white spot. The white spots grow bigger and run into each other over a few days and the little black dots just become more plentiful. After awhile the leaf dries out and falls off. What is happening and how do I stop it?

I found these yellow worm-like bugs on another infected leaf and was wondering if they may be the cause. They're about a 16th of an inch long.

Sorry for all the pictures, but I found an "adult" bug, I think. The picture is blurry because it's only about an 8th of an inch long.

That's a thrips. (The word always has an "s" on the end.) Yes, they can damage leaves. Direct hits of insecticidal soap work.

Thrips and possibly Tomato Spotted Wilt Virus - a virus the thrips carry.

Clean up all dead leaf litter in the area. Use a product called spinosad.
