My Hydrangea is dieing

My hydrangea is almost dead. It started with the leaves changing color. Instead of turning yellow or brown, like normally sick plants, the leaves turned from a normal healthy shade of green, to a very dark green, like boiled spinach. They then started to droop, and become soft and die off. The flowers that were health kept their color, put the stem where it attaches to the plant turned brown and the flower clusters fell off. I tried adding Hydrangea fertilizer, but that did not help.
