Disease on Tomatoes....soilborne?

My tomato plants have been looking worse and worse over the past couple weeks. While I'm certain at this point it could be blight or wilt, my questions are: Do these diseases tend to show up around this time of year? Are they likely in the soil to begin with, but don't attack until now? It seems I have been having this problem for years, late in the season.

Unfortunately I have not practiced crop rotation, due to the limited space I have. If I were to give the beds a break from tomatoes for a couple of years, would this problem still be waiting for the tomatoes when I continued growing again?
