Wall fireplace

Hi! How were the tv and fireplace installed on the rock wall? Before or after the rock installation? I have a rock wall in my basement and I'd love to add a fireplace like the one shown. I'm unsure about how to wire it since the rock is already on the wall.

    They are called pot lights. They are installed in a recessed can in the ceiling and screwed in.
