Unfortunately root knot nematodes are in the entire Florida soil, you can't escape from them. I can still get some decent harvest growing in the soil with them with good fertilizing.

The only way to avoid from them is grow your tomatoes in containers with fresh mix and don't add any existing soil from the ground or your raised beds to it.

As for your leaves, they look like septoria leaf spot. That's common here as well especially if the plants get too wet. My SunGold tomatoes in containers with fresh mixes managed to avoid them.

These are water soaked spots, this is how the all start, then it gets worse and basically disintegrates and dies.
