Tile lost it's grout & it's adhesive. To ensure tile to hold to wall, what do I do? Only regrout ? Or add adhesive? Only a few tiles fell.

If one fails than you have to assume they will all fail. You can do a quick repair to buy you a few weeks. It will hold just long enough for you to find a tile setter and new tiles. I would bet this whole thing needs to be done again...with new tiles.

The surface of the wall must be inspected for water damage before new tiles can be installed. It takes a LONG TIME for grout to "get lost". And even longer still for adhesive to fail. Something has been left for WAAAAAAAY too long.

Good luck. Bring in a tile setter who is well versed in these types of repairs....s/he will be worth every penny.

Where is the tile located? Is it a wet area like a shower?

"Behind the toilet" SCREAMS OUT water leak of some sort. I would bet dollars to doughnuts that this needs to be redone and the leak sorted out.

Well installed tiles do not "fall off" or lose their grout for no reason. Water leak behind the toilette (or behind the wall) is one reason for tiles to fail (and for grout to fail).

I think you might have a bigger issue than you think. Good luck. Photos would help.

Can condensation from toilet make tile fall?

My toilet does not get condensation. It can, however, cause tile, wall, and floor problems though if there is significant enough water from the condensation dripping on those surfaces.
