Help identifying a lawn creeper

Hi guys!


I've got a plant working its way across my lawn and I haven't had much luck identifying it. Any ideas?


Also can anyone suggest the best way of getting rid of it? The roots are quite long so forking it out would do quite a lot of damage to the lawn so am wondering about targetted poisons?
