Lime tree loosing leaves

My 3 year old lime tree is not and has not been doing well . Something is stripping the leaves and branches from the tree . The only thing i can see is little tiny blackish grayish larvae looking things as shown in pictures . Maybe they are not doing it and im growing it wrong ? I have never got one piece of citrus from the tree or never flowered etc .
Any help would be gratefully appreciated, I dont think the leaves are actually falling off but eaten . The 4th picture there is only 2 leaves that look like that with the miner leaf bug ?
