any idea where to purchase this holder?

A year ago I tracked down a woman in Canada who had a few left - they were about $40 each. If I could not have found her i was going to hire a handy man to make me one - it doesn't look impossible, but what do I know. Hope this helps, here is her email lianneproducts@rogers.com
