Squash Vine Borer questions

I suppose this could just as well be posted under Integrated Pest Management but I’ll try here first.

There's a few things regarding Squash Vine Borers that, so far, I’ve unable to find information on. Perhaps some of your experiences may shed light on these.

* Both, from what I've read and from personal experience, it seems Squash Vine Borers usually start their boring right around the base of the stalk at the soil line. When you find frass or similar damage elsewhere on the vine is it still likely to have been caused by a Squash Vine Borer or some another pest (cucumber beetle, etc.)?

* Regardless of where they start, do Squash Vine Borers tend to bore down toward the roots, outwards toward the stem ends or (as is the signs of the times) do they go both ways?

* Recently, I've preventatively sprayed (outer surface areas) with the BT-k. When I don't get Squash Vine Borer problems, I don't know whether the BT was effective or if the Squash Vine Borers just haven't shown up. The times I have definitely had them, I've injected BT into the stem which either took care of them or forced them to leave the stem where I dispatched them. I'm wondering if injecting BT into the stems right off the bat would be a more effective preventative treatment. This, of course, depends upon the number of squash plants (I usually grow less than six or so) and depends upon whether their activity actually does tend to be concentrated around the base of the plant.
