Rescuing an old hydrangea

There is a 15+ year old hydrangea in my dad's backyard that's been pretty pathetic for the last couple years. I'm not sure of the variety - it came with his house - but it is a good 7+ feet tall with cone shaped flowers. It is being defoliated by some sort of moth or insect, and this past spring only about a third of it even had leaves - there is lots and lots of dead wood.

Leaving the pest control issue for next year, would this plant grow back with a very severe pruning? He's not worried about blooms next year... he's worried about killing it.

Thanks.
