deciduous azaleas not looking good

Hi,

I have a mature 6ft+ deciduous azalea that bloomed beautifully this spring but since then the leaves are fewer, smaller and thinner compared to normal. Holly tone was added in spring. Maybe a cup.

The leaves look deficient. Can this be potassium deficiency? Any thoughts?

I have not tested PH in that area but typically my land is Acid enough. North Carolina zone 7b.
