Azalea dying off

Hi.



We have an azalea that has been here for many many years. It has been here so long I don't remember the name, plentiful, small purple flowers. It has always been very healthy until last year when a few branches began to die off. The rest of the plant appeared healthy and stable. Tilled the soil around and fertilised hoping to eliminate any further loss. I have attached a photo from this season where things started off promising but then the new blooms started to shrivel and died. Any thoughts on how to salvage the last stems? This one has extra special meaning and would very much hate to lose it.
