Estimating anything on a cost per square foot basis is likely to get you and your contractor into trouble. A better way is to look at cost data for bathroom remodels in your market and compare the cost and scope of those projects to what you have in mind.

Expect to pay lots. There is a shortage of good tradespeople and with the improving economy, it's going to get worse before it gets better. Employee poaching between contractors is going to go rampant, and the contractor you want is going to have to pay more to keep his guys. You'll have to pay him to do that.
