Rocker back out

I have a Poulan rider, Briggs and Stratton motor 31P677. I had a compression issue which was a head gasket, and I changed it out. I decided to adjust the valve lash, too, to .006 and .008. I must have done it poorly, because the rockers both backed out. The rocker stem for the exhaust valve (the one the rocker is mounted on), had backed out of the head. The intake valve rocker stem was not loose, but the rocker had backed out, and the pushrod was bent; I assume the nut with the torx set-pin had backed out. I am confused by both failures, and also wonder whether I should re-use the old head, since these stems are apparently set in aluminum. Any counsel?
