This floor drain most likely connected to the sewer drain and there could be a blockage down the line. I dealt with the same issues caused by old roots penetrating the sewer line or a blockage causing it to drain slowly and some of the water would back up and seep through the floor drain or seep through any void in the basement floor if there is an issue with the old drain pipe.

Your best bet is to have someone come out and use a video probe inspection camera and see if there is a pipe or drainage issue.

Exactly what GN Builders said. Have them run the drain, then use a camera on it.

Heads up, they will often try to sell you on an outside cleanout if you don't have one.

I was not willing to have my yard dug up and my clay tile drain disrupted to put in an outside cleanout. If I have to replace the drain line in the future, I will do it then. Instead, the put in a cleanout in the main stack in the basement.
