Hole where hosta was last year?

Last August we had to move a 2 + foot wide Elegans due to a new air conditioning unit going in. It was moved only about 4 feet.

I didn't do the digging, just gave instructions to my husband, as I was moving other smaller things. I did the after care, and it looked fine. Gave it extra water, usual post transplanting care.

But now I see there's a hole where the new planting was. The hole is at least 4" deep. I don't see any pips down in the hole, but I don't see pips for any of mine yet.

Is this likely to mean it died and the roots rotted? Since I didn't see the roots when he dug it up, I wonder if he dug a really small rootball. Thanks!
