It looks like a Lophocereus schotti monstrose form, and I'd (1) improve the soil that it's in / replant it in better yard soil (the Miracle-Gro C&S mix widely available is quite adequate - assuming you're planting it in Arizona and (2) while it's not the best time of the year to plant, you can help the plant by fertilizing it with Vitamin B-1/root stimulator / transplant juice, a week or so after you replant it.

Thank you, I just replanted this as I am trying to help it was like this when I got it. Sounds like I have planted it in the right soil and I do have some of the B1 I will give in a week or so as suggested. I suspected it was a Monstrose variety - again thank you.
