You could start woth an architect ot a very experienced and trusted GC. I would not hire just any GC, make sure they are well versed. I think the architect would be a good place to start.

I'm biased, but in my opinion you'll get the best result from a landscape designer who is skilled in structural design. A lot of GCs can't seem to see past the threshold of the structure, whereas a good landscape designer will have flow and integration from the house to the patio/porch to the backyard.
