Linear Fireplace proportions

Need a design for a linear fireplace, at 1 end of a long room. What are usual heights for mantels / shelve-hearths, does the center of the glass opening have to be centered to the fireplace ? I wanted to have it higher in the wall, so the fire can be seen from further - but now the mantel seems very high...
