Can the thyme and flagstone be placed right on the old grass? Besides watering, what maintenance is needed?

No, you would have to kill the lawn first. the stone is set on concrete pads or on sand, then the cracks are planted, in this case with thyme. Watering can be done via underground soakers or overhead pop ups.

Depending on the type of soil I'm dealing with, there are times when I will go the extra step by adding road base to prevent the stones from shifting. This is common with the clay soil in Santa Barbara.

Project where I used road base, sand and concrete for the stone base.
