Here in the Greater Vancouver area, a 54 inch pivot door would be between $4000 - $6000,
depending on the design and wood choice.
Most door manufacturers are more than willing to do custom work, so don't be shy to ask.
Ask to see their work, and get references. Good to see their work installed.

What type of grass or moss did you use between the pavers?

It is an artificial turf product by Dupont.

Obviously, very low maintenance for clients that are often on the go - which was the goal here!
It looks amazing - most visitors reach down and touch it as it looks so real.
Turf products have come a long ways!
