Cactus ID and Advice Pretty Please

My wife brought this cactus home a few days ago and I am not sure what type it is.

1.) If you can help ID it would be appreciated.

2.) It looks to me like this needs a larger pot ASAP (Concur?).

3.) Are the brown spots between the spines a concern and is there anything I can do to get rid of it and/or prevent it going forward?
