Refreshing soil in a potted clematis

I have a clematis 'President' which has been in a pot for about 5 years. I think its potting medium has likely degraded to the point where it is too compacted and stays too wet. With other woody potted plants that I have, I bareroot them, and move them to fresh 5-1-1 mix, when needed. I know that clematis hates to have its roots disturbed, so how does everyone handle the issue of the potting soil getting degraded and compacted over time? Do you transplant your potted clematis on a regular basis?

Soil will break down over time and will have to be replaced, I replace soil by 1/2 or more every couple years with mine that are in 15 gal pots, I have 25 plus and 8 or 9 are in 15 gal pots and have been for many years and the others are all in the ground. Finished with all my prunning a couple days ago, I'm so ready for clem bloom time:)

How do you do the refreshing? Do you unpot the clematis, knock off the soil at the sides and bottom, and replace that? Cut wedges in the rootball and replace that soil? What "half" is getting replaced?

I'm not too concerned about Clematis hating root disturbance. That would be news to me. They certainly hate root competition in the garden but I unpot and then repot with fresh soil those I have in pots all the time.
