Refreshing soil in a potted clematis

I have a clematis 'President' which has been in a pot for about 5 years. I think its potting medium has likely degraded to the point where it is too compacted and stays too wet. With other woody potted plants that I have, I bareroot them, and move them to fresh 5-1-1 mix, when needed. I know that clematis hates to have its roots disturbed, so how does everyone handle the issue of the potting soil getting degraded and compacted over time? Do you transplant your potted clematis on a regular basis?
