When and how to plant balloon flower/platycodon?

I have a bare root platycodon aka balloon flower. It came in the mail and is in a plastic bag with a bit of soil, no instructions and I'm not sure how or when to plant it (zone 6b, southwestern PA) other than it will need to be re-hydrated for a few hours before I plant it.
