Need Help Reflooring Kitchen

We're ripping out our entire kitchen including the floor and subfloor. Pre-remodel the floor is old linoleum tile and the countertops are formica. Post remodel the floor will be porcelain tile and some of the countertops will be granite. We understand that the current structure may need to be reinforced to hold the new tile/countertops, but we won't know that until we pull everything up. My questions are the following: Do we need to put down a subfloor, a leveler, and then backerboard (and what thicknesses) or do we put down something that will work as both the subfloor/backerboard and what thickness should that be?
