Starting lawn from scratch

So I am doing a total lawn rehab. 1,000 SF of the lawn died over the winter from leaves and moss smothering it to death. The other 400 SF I have uncovered from about 5 inches of bark mulch, weed fabric and landscape rock.

I have a rotatiller coming on Saturday to work the soil.

It seems like there are so many options- compost, lime, humic acid, top soil..... Do I add these things between rounds of tilling the soil or should I wait until I have leveled the soil and added top soil where needed to level? Do I need a soil test in order to be successful? Any other tips?

Thank you.
