Pruning growth at the top will have no effect on the bottom and middle areas. The usual cause for growth at the top and bare areas lower down is too little sun. Drought conditions may be a factor as well, but rhododendrons will reach for the light like a great many other plants. This tendency varies somewhat from variety to variety, but unless there is some way of letting more sun reach your plant, there is not much you can do about the bare areas.
