HI -- Look for someone in your area that fixes such things . Looks like a very old block foundation that is now failing . Do you have water coming in your basement ? Be prepared for some expensive sad repair costs . I hope it will be an easy fix for you , but I think this has been a problem for a long time and was covered up with surface repairs . Your home inspection should have found this when you bought the house .

Looks as though the block patio might not have a proper footing to allow for freeze and thaw. The patio can be stabilized. Look for a company that can lift and level cracked driveways by using expanded foam and metal supports. You won't know how much until you get a quote. Looks like the patio is not too far gone, so should be pretty easy.
