looks like exactly the reverse of what you pictured: yours is blue glass tile and natural stone, where i think the one above is brown/cream glass tiles and blue slate stones.

I could have sworn it was part of Home Depot's selection that was this type of mosaic for like under $7, but I looked online and don't see that exact tile. They have this one at Overstock.

It doesn't look familiar but if you bring the photo to your local tile shop they should be able to find something similar. I tried searching "random linear mosaic tile blue" but couldn't find anything exactly like it.
