If it makes you feel any better, it appears from the photo that it had a produced a terminal spike. It may not have survived even with the best care

Edit: just noticed the spotted leaves. Not a phalaenopsis, more likely a paphiopedilum. I retract my statement about the terminal spike

Yes, it was a Maudiae type Paphiopedilum. Maybe have another try after finding some complete cultivation notes written for wherever you are in the USA.

This one is a goner but...

It should have put out new growths from the base. Paphs generally flower once from each growth and then that growth produces one or more new growths (side shoot/basal type) for next year's flowers. Also, in my experience is not unusual for older paph growths to die off after 2-3 years.

To help figure out what happened, we need to know: What exactly does 'watered regularly and cared for it appropriately' involve as far as: How often was it watered, what type of media was used and what type of light was given?
