I've worked on this same scenario for many of my clients and 9 times out of 10 I specify built-in bookcases.

The help give the space character and depth. They can be done at a depth of 12" minimum and can be designed in a sleek contemporary style such as the ladder style you mentioned. I think a deep weathered oak finish would be beautiful.

I think they would be an asset to the room. I might also suggest painting the fireplace wall bump out an accent color, whether its an actual color or a richer neutral or grey to work with your existing palette.

What would you do on top of the cabinets? The ceilings are 9 feet

I would not do built in's because they can give a bulky and oversized look. I like the idea of mirrors or paintings with a small dresser/chest underneath. Or if you want to display books and decorative items there are plenty of leaning shelvings or open shelvings available. CB2 has some nice shelving units that attach to walls and have a very airy and open feel. The one below is acacia wood but they have multiple ones, white, grey, etc.

If you can, back the couch up a foot or two. Your coffee table is very heavy and bulky looking and it all looks quite crowded.
