Advice on a rescue phal?

My mom is teacher and always gets gifts around the holidays from students, and one of them gave her a cute little phal, probably just from the grocery store or whatever. She totally neglected it and it had probably gone a month without water when I saw it and took it.
I've never had an orchid, but started reading about them. It was in spag, and seemed quite soggy whenever I watered; I just got some bark to move it into which is soaking now. When I pulled it out of its pot it was a disaster! Lots of gross "hollow" roots, which I cut off, and what's left doesn't look especially healthy to me. I'm not sure what else should go, or if it's okay the way it is? There's lots of yellow, and there was also something weird and bright green in the moss? Not sure if it was fertilizer of some sort or something?


I have it just sitting out for right now, but I'm not sure what I should do to help this little guy!
