The door was custom made by a shop here locally that is no longer in business. However I'm currently using a shop named KSR doors in Pleasant Grove that does even better work. You can reach the owner of KSR Doors, Kurt, at 801-362-8984.

3' 6" door width by 8' tall with 15" wide side light and a 15" transom window built over the top. I had Stock Building Supply and several other door companies, bid the original door at $6800 to $7900. I was able to negotiate this door custom built for just under $4k. Kurt would likely be less than that based on what I typically am charged for similar doors.
