Sick camellia

My "Camellia Japonica" (variety unknown but it has red flowers) is about 15 years old; is planted in an azalea/camellia potting mix in a container; and is growing in filtered shade/partial sun.

Here are photos of the shrub, stems and leaves.



What is causing the poor growth and sickly leaves?

Can I bring the plant back to health or should I replace it with a new camellia?
