perrennials in raised beds

I have a question regarding soil in raised beds. I built 2- 24" high beds for my mother in law who has trouble bending. Problem is, I planted rhubarb in one bed and asparagus and grapes in the other. However after 4 years the soil has lowered over 12" forcing her to bed into the bed. They were filled with 70% soil and 30% compost. I was told it was the perfect raised bed mix. I'm now forced to uproot everything and start again. How do other people prevent this from happening? I see Youtube video of people growing perennials all the time? What have I done wrong? Thanks
