What happened to my succuent?

Can anyone tell me what happened to my Graptosedum. Split stem and leaves just falling off to the touch.

You can re-root those rosettes on the tips. Let the cuts dry and place in bright shade in some gritty mix. Be patient. They will grow. Begin watering when they have roots.
